The Health Insurance Claim Playbook

File It Right, Fight It Written, Get It Paid — Under India's New IRDAI Rules

Roughly 1 in 12 health insurance claims in India is repudiated, and complaints on IRDAI's Bima Bharosa portal crossed 73,000 this financial year. Most families lose money they could have won — because nobody told them the rules. This 19-page playbook is those rules: the 1-hour cashless decision, the 3-hour discharge approval, the 30/45-day settlement deadlines, the 60-month moratorium, 12 rejection reasons decoded with your counter, the complete free escalation ladder, and 6 fill-in templates. Every fact verified against IRDAI's 29 May 2024 Master Circular and official sources, current to 27 September 2026.

Who This Book Is For

If your family pays a health insurance premium, this is your rulebook

👨‍👩‍👧

Families With a Floater Policy

One hospitalisation away from testing a ₹5–10 lakh policy. The Day One Audit and Emergency Claim Card make you claim-ready in one evening.

💢

Anyone With a Rejected Claim

Repudiated, delayed or partially paid? Get the counter to each of the 12 most common rejection grounds, plus the appeal letter that demands the clause and the committee approval.

👴

Children of Senior-Citizen Parents

You manage their policy and their hospital runs. The escalation ladder, portability rules and moratorium protection are what keep their cover intact.

🧾

First-Time Policy Buyers

Buy right the first time: waiting periods, sub-limits, the non-disclosure trap and the free-look period — before a claim ever tests your choice.

What You'll Master

Seven defences, from proposal to payout

⏱️ The Deadline Rulebook

Cashless decided in 1 hour, discharge in 3, reimbursement in 30 days, investigation 45 — with interest at 2% above bank rate when insurers cross the line.

🛑 The 12 Rejection Counters

Non-disclosure, "OPD was enough", no active line of treatment, room-rent breaches, lapsed policy — each with the exact argument that reverses it.

⚖️ The 60-Month Moratorium

After 5 continuous years your policy cannot be questioned except for proven fraud — the single rule that defeats the most common rejection of all.

🪜 The Free Escalation Ladder

Grievance Redressal Officer (15 days) → Bima Bharosa → Insurance Ombudsman (award in ~3 months, ₹5,000/day penalty for ignoring it) → consumer commissions.

✍️ 6 Fill-In Templates

Rejection appeal, delay reminder with interest demand, Bima Bharosa grievance, Ombudsman complaint, hospital records request — copy, fill, send.

🚨 Emergency Card & First 60 Minutes

A printable family claim card plus the minute-by-minute sequence for the hour after an emergency admission — when nobody remembers policy numbers.

Inside the Book

8 parts — from the numbers to the templates, with sources

  1. Why Claims Fail — And Why That Is Fixable

The verified numbers behind India's health-claim problem, and the 2024 IRDAI reforms that became your unfair advantage.

  2. Know Your Policy Before You Need It

The 30-minute Day One Audit: the Customer Information Sheet, waiting periods, sub-limits, the non-disclosure trap — plus the fill-in worksheet and 10 myths debunked.

  3. Filing a Claim the Right Way

Cashless (planned and emergency), the 1-hour/3-hour rules, what to do when cashless is denied, the reimbursement sequence and the full document checklist.

  4. When Things Go Wrong

The 12 rejection reasons with your counter to each, the worked ₹3.2 lakh case that came back, and how room-rent arithmetic actually eats money.

  5. The Escalation Ladder

GRO → Bima Bharosa → Insurance Ombudsman → consumer commissions: eligibility, deadlines, what each rung feels like, and the mistake that kills good cases.

  6. Ready-to-Use Templates

Six fill-in letters plus the Family Emergency Claim Card and the first-60-minutes admission protocol.

  7. Keep the Protection You've Earned

Renewals, grace periods, portability's 72-hour/5-day rules, and the no-claim bonus — plus the one-page "if this happens" triage map.

  8. Every Deadline on One Page

The cut-out-and-keep table of every clock in Indian health insurance — cashless, settlement, grievance, Ombudsman, grace and moratorium.

Frequently Asked Questions

Everything you need to know before buying

My cashless request was denied at the hospital. Is my claim rejected?

No — and this is the most expensive misunderstanding in Indian health insurance. A cashless denial is only a refusal to pre-authorise, not a repudiation. Take the denial note in writing, pay the hospital, collect the discharge summary and itemised bills, and file the same claim as a reimbursement claim, which the insurer must decide within 30 days of your last document. Many cashless denials on thin grounds are paid on reimbursement.

Is the content current and verified?

Yes. Every rule, deadline and statistic was verified in September 2026 against the IRDAI Master Circular on Health Insurance Business dated 29 May 2024, the IRDAI (Insurance Products) Regulations 2024, the Cashless Everywhere circular of 31 January 2024, IRDAI's official policyholder pages, the Council for Insurance Ombudsmen, Finance Ministry data presented in Parliament, and the Insurance Ombudsman framework. The full source list is printed in the book.

My claim was already rejected. Will this book still help?

That is exactly what Parts 4 and 5 are built for. You get the counter to each of the 12 most common rejection grounds, a fill-in appeal letter that demands the policy clause and the Claims Review Committee approval date, and the complete free escalation ladder — Grievance Redressal Officer (15 days), Bima Bharosa portal, and the Insurance Ombudsman, which is free and passes awards within three months.

Is escalating to the Ombudsman expensive or complicated?

It is completely free — no fees, no lawyer needed. You file a written, signed complaint (a fill-in template is in the book) at the Ombudsman centre with jurisdiction over your address, within one year of the insurer's rejection or unsatisfactory reply. Mediation recommendations can come within a month and awards within three months; ignoring an award costs the insurer ₹5,000 per day.

Does the book cover buying a policy, or only claims?

Both. The 30-minute Day One Audit worksheet and the non-disclosure trap section prevent disputes before they start, and Part 7 shows how renewals, the grace period and portability protect the waiting periods, no-claim bonus and the 60-month moratorium you have already earned when you buy, renew or switch policies.
